Premature Ejaculation and Erectile Dysfunction

Integrative management of Andrological and Urological diseases utilizes TCM theory and clinical experience in the management of male-related disorders. Male-related Andrological dysfunction such as Erectile Dysfunction and Premature ejaculation, can be divided into psychological and functional causes. Psychological causes are frequently caused by excessive anxiety, depression, poor relationships or external stressors. These affect the emotional well-being of a person prior to sexual activity resulting in poor performance. Functional causes are usually caused by existing underlying conditions. These include conditions such as hormonal, vascular, neurological and iatrogenic causes.

Causes of PE and ED

Hormonal causes include hypogonadism, low testosterone and thyroid abnormalities. Vascular causes include Diabetes Mellitus, Atherosclerosis, High Cholesterol or Chronic Kidney Disease (CKD). It results in the hardening of blood vessels causing difficulties with the blood flowing to the penis. Neurological causes include Diabetes Mellitus, Vitamin B12 deficiency, Cauda Equina Syndrome, Spinal Cord Injuries, Guillain Barre Syndrome and Trauma. Iatrogenic causes refers to patients who underwent a procedure or surgery around the penile area such as radical prostatectomy for Benign Prostatic Hyperplasia (BPH) or prostate cancer where the surrounding nerves supplying the penile region is being damaged. These disorders in turn destroy the normal functioning of nerves innervating the penis, causing difficulties in the conduction of the sensory and motor systems. As the nervous system is damaged, the blood pressure within the penis is unable to be controlled, hence resulting in conditions like erectile dysfunction.

Management of PE and ED

Treatment of Andrological disorders requires the proper diagnosis of underlying conditions which will be done with our medical doctors and TCM physicians. A detailed consultation will be done to understand the underlying reason for your condition. Ultrasound and blood test might be taken to understand your blood sugar levels as well as the vascular flow to the penile region. Low Libido (Reduced Sexual Drive)

Causes of low libido

The causes of low libido are multifactorial. Hormonal imbalances such as low testosterone, elevated prolactin, or thyroid dysfunction can play a role. Chronic medical conditions—including diabetes mellitus, cardiovascular disease, chronic kidney disease, and chronic pain—are also contributing factors. In addition, psychological conditions such as depression, anxiety, and heightened stress can significantly impact sexual drive.

Management of low libido

TCM offers a holistic approach to managing low libido. Certain herbal therapies, including safflower, ginseng, ginkgo biloba, and horny goat weed, are traditionally regarded as aphrodisiacs. Modern research suggests these herbs may support libido by improving hormonal balance, enhancing erectile function, and increasing the release of nitric oxide. This nitric oxide–mediated vasodilation of penile blood vessels facilitates erection, thereby addressing both desire and performance. In addition, electroacupuncture has also shown to increase the peripheral blood flow to the penile region through 6-point electroacupuncture technique.

Delayed Ejaculation and Anorgasmia

Delayed ejaculation (DE) and anorgasmia are male sexual dysfunctions characterized by persistent difficulty or inability to achieve orgasm despite adequate sexual stimulation and desire. These conditions are less common than erectile dysfunction or premature ejaculation but can lead to significant psychological distress and interpersonal difficulties.

The prevalence of delayed ejaculation is lower compared to other male sexual disorders, with estimates ranging from 1–4% in the general population. However, prevalence may be higher in men with comorbid medical conditions such as diabetes mellitus, neurological disease, or those receiving long-term pharmacological treatment with antidepressants or opioids.

Causes of DE and Anorgasmia

The etiology of delayed ejaculation and anorgasmia is multifactorial, involving anatomical, neurological, endocrine, psychological, and pharmacological components.

i) Anatomical factors: Congenital or acquired ejaculatory duct abnormalities that impair semen emission.

ii) Neurological factors: Peripheral and central nervous system dysfunction, including multiple sclerosis, diabetic neuropathy, and spinal cord injury or trauma.

iii) Endocrine factors: Hormonal imbalances such as hypogonadism, hyperprolactinemia, thyroid dysfunction, or Cushing’s disease.

iv) Pharmacological factors: Use of selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants, opioid analgesics, and some antihypertensives.

v) Psychological factors: Depression, anxiety, performance anxiety, and relationship difficulties.

Management of DE and Anorgasmia

Treatment is directed at the underlying etiology and often requires a multidisciplinary approach:

Endocrine causes: Hormonal replacement or regulation via different TCM herbs can be done to restore the balance

Neurological causes: Supportive care and targeted interventions; electroacupuncture has demonstrated potential in improving neurogenic delayed ejaculation by enhancing peripheral blood flow and modulating neural pathways.

Psychological causes: Certain herbs and herbal formulas have been shown to improve the mental state of individuals. In addition,sex therapy, cognitive-behavioral therapy, and couples counseling can also help.

Pharmacological causes: Requires the adjustment of offending drugs or substitution of offending medications.
